v3.8.0 — think() decision-router integration + timeExtension variable-order fix + dead-code cleanup

- [FIX] think() Step 13: needsCrisis/needsSilence/isFableBlocked moved before timeExtension
  (bug: variables defined after timeExtension block, guard condition never evaluated)
- [FEAT] think() now calls decisionRouter.evaluate() after 13-step analysis pipeline,
  injects cognitive decision (pause/accelerate/turn/hold/heal/resonate/transmit/rest)
  into return value as decision field
- [CLEANUP] Removed 41 upgrade temp files (src/core/upgrades/)
- [CLEANUP] Removed old heartflow-engine.js, identity/src/, memory/src/ (migration residue)
- [FIX] judgment.js/metaMemory.js: meaningful-memory require path corrected
- [AUDIT] Zero /Users/apple/ privacy leaks, zero syntax errors
- [VER] v3.7.1 -> v3.8.0
